  Processing charges for issue of Blood/Blood components to the patients admitted in private hospitals/autonomous bodies/nursing homes/private ward of Govt Hospitals/Private ward of Govt. Blood Banks and processing charges from Govt hospitals
  
  It is mandatory to get the Blood requisition form which has to be stamped by blood bank of hospital or if blood bank is not available in the hospital, it should be stamped by the doctor treating the patient and the hospital along with the blood sample of the patient
  
  The person collecting the blood must possess the valid Govt ID card and proof of relationship with the patient.

The request could be sent to The Secretary General, Indian Red Cross Society, 1 Red Cross Road, New Delhi-110001 along with Demand Draft drawn in favor of St. John Ambulance payable at State Bank of India, Main Branch, New Delhi.

i) Name and contact details of Branch
ii) Delivery address for the blank forms
iii) DD amount, DD number and name of bank
iv) Type of form required
v)  Number of forms required
 

1) Form A/4-S(P) Senior (Professional) - First Aid (FA)/Home Nursing (HN)/Mother Craft (MC)/Hygiene & Sanitation (HS)/Meckenzie (MS)
Validity of Senior Professional Certificate is 3 years
To be printed on A4 size paper

2) Form A/4-V Voucher (V),  Medallion (M). Label (L) - First Aid (FA)
Validity of Medallion and Label Certificate is Life time and of Voucher Certificate is 5 years
To be printed on A4 size paper

3) Form A/4-S(S) Senior (Student) and Junior - First Aid (FA)/Home Nursing (HN)/Mother Craft (MC)/Hygiene & Sanitation (HS)/Meckenzie (MS)
To be printed on A4 size paper

4) Form for Lay Lecturers training course Home Nursing and First Aid
Validity of  Lay Lecturer Certificate is 5 years
To be printed on A4 and A3 size paper
